A non-refundable $450.00 registration fee, as specified in the tournament announcement, is due at the time this form is submitted. Make checks payable to Latham Lassies Softball League and mail check and form to: Latham Lassies Softball League, P.O. Box 105, Latham, NY 12110.

Registration is on a first come, first serve basis, with your check holding a place for your team.

Official ASA Team rosters, proof of insurance, photo ID, and birth certificates must be in the tournament director's hands 10 days prior to the start of the tournament, unless he/she advises otherwise.

Any team that wants to participate in District 7 Tournament Play must have their coaches (any dugout personnel) become ACE Certified for 2012. All managers & coaches must be dressed alike, this includes shirts and pants in team colors. NO jeans or cut offs will be allowed on the field!
